Specimen Labeling Requirements
To help ensure the best service and patient care, every specimen submitted must be clearly labeled with a minimum of two unique patient identifiers that match the order.
Acceptable identifiers include the following:
- Patient's full name
- Date of birth
- Patient ID number
- Medical record number
- Order number
